Asbestos certification application procedures

Effective March 15, 2004, applications to the New York City Department of Environmental Protection Asbestos Control Program (ACP) for asbestos certification must include the applicant’s New York State Department of Motor Vehicles (NYS DMV) identification number.

The ACP applications and instructions have been updated and only the new forms shall be accepted for processing. The instructions include information for getting a photo image at NYS DMV and obtaining the 9-digit number.

The new applications and instructions are posted on the Department’s Web site, at www.nyc.gov/dep. Packages may also be picked up at the ACP, 59-17 Junction Boulevard, 8th Floor, Flushing, New York.

We are currently working on a system for applicants to receive their certificate identification cards in the mail from the NYS DMV. The cards will be sent to the address listed on the ACP application. Further information regarding this matter will be supplied as it becomes available.
